Social Care Network
A Social Care Network (SCN) is a group of service providers addressing Health-Related Social Needs (HRSN), including community organizations, Medicaid managed care organizations (MCOs), healthcare providers, and other partners. Participants in the network enter into contracts and are supervised by an SCN Lead Entity. Organizations within the SCN can assess and refer Medicaid members with unmet HRSN needs to appropriate services.

Following the screening, we'll let you know what you qualify for and assist you in accessing those services.
